Is the KDP interior formatter really free?

Yes. It runs entirely in your browser, needs no account, and adds no watermark. DraftToDone sells a paid pipeline for producing whole books; formatting one manuscript is not part of it.

Which trim size does it produce?

6 × 9 inches, the most common trade paperback size on KDP. Other trims are not supported yet.

Will Amazon accept the PDF it makes?

It applies Amazon's published margin, gutter and resolution rules, and warns when your manuscript breaks one. No tool can promise KDP approval, and this one does not.

What does it need from my .docx?

Chapter titles styled as Heading 1. Without them the chapter breaks are guessed, and the tool tells you when that happened so you can check the list.
